PSA Norway Makes Orders Following Investigation into Statfjord A Leak

On Saturday, May 24, 2008, a hydrocarbon leak occurred in one of the shafts of the Statfjord A facility in the North Sea. The incident could, in our opinion, have resulted in a major accident under marginally different circumstances. On the basis of our findings during the investigation, we have issued orders to StatoilHydro, Industrikonsult AS (IK) and Aker Solutions AS.

We have issued the following order to StatoilHydro:

Pursuant to Section 3 of the Management Regulations relating to management of health, environment and safety and Section 5 of the Framework Regulations relating to responsibility pursuant to the first subsection of these regulations, cf. Section 58 of the Framework Regulations relating to administrative decisions, StatoilHydro is ordered to review its routines for similar work operations, identify improvement measures and implement them no later than November 1, 2008.

The company shall furthermore review and evaluate its management of modification projects to identify and implement additional measures to improve management of such projects. This shall be done no later than December 1, 2008.

Implemented measures shall be followed up and evaluated to ensure that the desired effect is achieved. The work shall take into account, but not be limited by our investigation report following the incident.

We must be notified when the order has been complied with.

We have issued the following order to IK:

Pursuant to Section 3 of the Management Regulations relating to management of health, environment and safety and Section 5 of the Framework Regulations relating to responsibility pursuant to the first subsection of these regulations, cf. Section 58 of the Framework Regulations relating to administrative decisions, Industrikonsult AS is ordered to identify and implement measures to improve its management of activities, including compliance with applicable requirements for development and qualification of equipment and methods, ensuring that management and executing personnel have the necessary competence and identifying and handling risk in connection with the execution of assignments.

The deadline for complying with the order is set at December 1, 2008.

We must be notified when the order has been complied with.

We have issued the following order to Aker:

Pursuant to Section 3 of the Management Regulations relating to management of health, environment and safety and Section 5 of the Framework Regulations relating to responsibility pursuant to the first subsection of these regulations, cf. Section 58 of the Framework Regulations relating to administrative decisions, Aker Solutions is ordered to identify and implement necessary improvements in the company's management of modification assignments, including identification of risk and use of risk information during the planning and execution of high-risk work operations, including choice of work methods and equipment and follow-up of sub-contractors.

The deadline for complying with the order is set at December 1, 2008.
